Services
As a PACE participant, you receive all the care and services covered by Medicare and Medicaid, as authorized by our Interdisciplinary Team, as well as additional medically necessary care and services not covered by Medicare and Medicaid. Preventative care is covered and encouraged as the focus is to help you live in the community for as long as possible.
The professionals on our Interdisciplinary Team are experts in working with older people. They work together with you and your family to develop your most effective plan of care.
Services include but are not limited to:
- Adult Day Services
- Transportation to the PACE Center and medical appointments
- Home Care
- Primary Care
- Prescription Drugs
- Emergency Services
- Physical Therapy
- Occupational Therapy
- Dentistry
- Social Services
- Nutritional Counseling
- Hospital Care
- Nursing Home Care
Services are provided in the home, in the community and at the PACE Center.
Your family members and other caregivers will be supported with training, support groups and respite care to help families keep their loved ones in the community.
Immanuel Pathways is a provider-sponsored health plan. This means your PACE physician and other care providers are also the people who work with you to make decisions about your care. No higher authorities will overrule what you and the PACE team agree is best for you.
PACE participants must receive all needed health care, including primary care and specialist physician services (other than emergency services) from the PACE organization or from an entity authorized by the PACE organization.
PACE participants may be fully and personally liable for the costs of unauthorized or out-of-PACE services. Emergency services are covered.

We are proud to offer Lifeline by Immanuel, a medical alert service that offers the peace of mind of knowing help is always available.
Costs
Immanuel Pathways charges no deductibles or copayments for any drug, service or care approved by the PACE interdisciplinary team.
If you are eligible for Medicaid and Medicare, you pay no premium for the Immanuel Pathways services.
If you are eligible for Medicaid only, you pay no premium for Immanuel Pathways services.
If you are eligible for Medicare only, you will be charged a monthly premium for the program and a premium for Medicare Part D.
If you are not eligible for Medicare or Medicaid, Immanuel Pathways staff will discuss a private pay rate with you.
