Become a Participant
You can enroll in Immanuel Pathways if you meet all of the following criteria:
- You are 55 years old or older.
- You live in Harrison, Mills or Pottawattamie County in Iowa.
- You are assessed to meet nursing facility level of care.
- You are able to live safely in the community with the support of Immanuel Pathways services.
