Immanuel Pathways is a non-profit organization offering a unique service to enable people with chronic care needs to be served in their homes and communities whenever possible.

Our comprehensive team of health care professionals helps coordinate your health, your connection with the community and the life you’ve built in your home.

The model of service is called PACE:  Program of All-inclusive Care for the Elderly.

PACE is a program for older adults and people over age 55 living with disabilities.This program provides community-based care and services to people assessed to need nursing facility level of care. PACE was created as a way to provide you, your family, caregivers and professional health care providers flexibility to meet your health care needs and to help you continue living in the community.
